1. “Normal” Lab Ranges Weren’t Designed to Tell You Whether You Feel Your Best
Most people assume lab ranges reflect what is healthy, but that’s not actually how they’re created. These ranges are calculated by averaging the results of everyone who gets lab testing, and unfortunately, most people getting labs drawn are not functioning at their best. A result that falls somewhere within that broad population average is labeled “normal,” even if it is far from where your body feels balanced, energized, and well.
This is why someone can be told their thyroid is “normal” despite deep fatigue, or that their vitamin D is “fine” even while they struggle with low mood or immune issues. In functional medicine, we use optimal ranges, which are narrower and based on where people feel their best, not just where they avoid a diagnosis. When your symptoms tell a different story than your labs, we listen to your body first.
2. Hormone Fluctuations Often Hide Behind Normal Labs
This is especially true in perimenopause, the wildly dynamic phase that can begin as early as your late 30s. Hormones in perimenopause don’t follow a stable pattern. They rise, fall, surge, and crash from week to week. A single blood draw can catch one moment in that rollercoaster and completely miss the bigger picture.
So even if your estrogen or progesterone levels land “in range,” those numbers don’t reflect the ongoing hormonal instability that causes symptoms like night sweats, mood swings, anxiety, heavy periods, breast tenderness, and insomnia. Many women are told nothing is wrong because their hormones happen to look passable on the exact day they were tested, yet their lived experience tells a very different story.

The thyroid is one of the most commonly misunderstood areas of testing. In many traditional settings, the only marker checked is TSH, a pituitary hormone that reflects how loudly the brain is asking the thyroid to work. But TSH doesn’t tell you how much active thyroid hormone is available to your cells, how well your body is converting T4 to T3, whether inflammation is impairing thyroid function, or whether antibodies are silently attacking the gland long before changes appear in TSH.
Women can experience classic thyroid symptoms (fatigue, hair shedding, constipation, weight gain, brain fog, cold intolerance, anxiety) even when their TSH falls within the standard range. This is why we run a full thyroid panel, including free T3, free T4, total hormones, reverse T3, and antibodies. A “normal” TSH is only one piece of a much bigger, more clinically meaningful picture.
4. Gut Dysfunction Affects the Entire Body, Even Without Digestive Symptoms
Many people assume that if they aren’t bloated or constipated, their gut must be fine. But the gut influences far more than digestion. It plays a major role in hormone balance, immune function, inflammation, detoxification, metabolism, and even your brain chemistry. You can have significant gut dysfunction without any discomfort in your abdomen.
Low energy, brain fog, skin issues, anxiety, stubborn weight gain, and inflammatory symptoms often trace back to imbalances in the gut, even in women whose digestion is "normal". Blood sugar swings, poor sleep, undereating, overexercising, chronic inflammation, and nutrient depletion all place strain on the HPA axis and metabolic pathways.
